Sample Program

We are pleased to provide the Alpha-1 community with the option of trying Zemaira® at no charge for up to 2 weeks. To obtain a Trial Pack of Zemaira® for one of your patients, simply complete the Zemaira® Product Sample Request form PDF and fax it to our Customer Support group at the number shown on the form. Please note that Zemaira® samples can be requested only on a per-patient basis.

Should you decide to continue your patient on Zemaira®, one of our Specialty Pharmacy Partners, Accredo Therapeutics and Coram, Inc., (a division of Aprial Healthcare) can provide you with assistance. You will need to obtain the necessary insurance clearance and set up an infusion service. Because it can take up to 4 weeks to complete this process, we encourage you to work with your patient to fill out the referral form and fax it to the selected specialty pharmacy at the same time you fax us the Product Sample Request. That way, all the paperwork necessary for your patient to continue on Zemaira® should be completed by the end of the trial period.
